Of course, you don't need a succession crisis to start a civil war for the throne. A typical Roman method, for example, was to have the troops declare a new emperor and march on Rome/Ravenna/Constantinople and try to depose the old emperor.
Then again, many medieval succession crises came about by nobles or pretenders to the throne engineering a succession event (i.e. kill the king) when they thought they had a good chance at winning. Whether or not there was a legitimate heir was not necessarily important (and if it was, you can kill or disqualify them as well).
